Please do not send samples without first
making contact with Dr. Moon-Fanelli. Different blood collection tubes are required
depending on whether your dog will be taking part in one or both study groups!
Both the consent form and the survey must be filled out and
included for each dog.
This is a 19 page Adobe PDF file containing the information

Instructions
To copy the forms to your hard disk, right-click the link and

Please Also Consider...
If you are taking your dog in for the blood draw to send a
sample for the Bull Terrier Tail Chasing study, please consider also having a
sample drawn and sent at the same time for the Lethal Acrodermatitis study,
which also needs samples from both normal and affected Bull Terriers. More
information on that study can be found on the Bull
Terrier Club of America site.
